Gell and Coombs

In 1963, two British immunologists, Robert Coombs and Philip Gell, published in a book a new scheme for classifying different types of hypersensitivity reactions. Although knowledge on the underlying immunological processes has greatly expanded over the past 50 years, this simple classification is still used nowadays Hypersensitivity refers to undesirable reactions produced by the normal immune system, including allergies and autoimmunity. They are usually referred to as an over-reaction of the immune system and these reactions may be damaging, uncomfortable, or occasionally fatal. Hypersensitivity reactions require a pre-sensitized state of the host. The Gell and Coombs classification of hypersensitivity is the most widely used, and distinguishes four types of immune response which result in. Gell and Coombs classified hypersensitivity reactions into four 'types'. I suggest that the premise that these reactions represent 'hypersensitivity' manifestations is limiting and that they represent four major strategies that the body uses to combat infectious agents. The Gel and Coombs classification is widely used to characterise hypersensitivity reactions. It is based on the DELAY between exposure to antigen and onset of hypersensitivity reaction Robin Coombs was the last survivor of the distinguished group of immunologists that included Philip Gell, John Humphrey, John Marrack, Peter Medawar and Robert White and who were responsible for the renaissance of British Immunology after the Second World War Throughout this book we have attempted to use an unequivocal and precise terminology, in which the 'allergic state' is taken to denote a state of altered reactivity to an antigenic substance. A main stumbling block to clear thinking has always been the use of the words 'immunity' and 'immune' - implying as they do by their common meaning absolute protection against a noxious.

1. Introduction. In the 1960's, the British immunologists Coombs and Gell proposed a classification of allergic reactions underlying disease (Gell and Coombs, 1963) that they thereafter applied to the particular area of drug hypersensitivity reactions (Coombs and Gell, 1968).The Gell and Coombs's classification, as it is still known, became the absolute reference for defining the pathogenesis. The Gell and Coombs's classification divides drug allergies into four pathophysiological types, namely anaphylaxis (type I), antibody-mediated cytotoxic reactions (type II), immune complex-mediated reactions (type III), and delayed type hypersensitivity (type IV)

An important thing to remember is that the presence of a positive coombs' test in the lab does not necessarily result in hyperbilirubinemia in the infant. The risk of needing phototherapy is certainly greater, but there are many factors impacting bilirubin levels, and assessment of all of these elements is critical to making an appropriate decision about treatment notes65 Gell and Coombs Classification of Hypersensitivity Responses Hypersensitivity reactions are immune responses that cause tissue injury and morbidity for the host.
